Waiver & Release of Liability
Assumption of Risk: I acknowledge that participation in soccer activities and use of facilities involves inherent risks, including injury, physical contact, collisions, falls, and other athletic hazards.
Release of Liability: I release Canyon Academy FC, its coaches, staff, volunteers, and affiliates, as well as Vista College Preparatory, its governing board, officers, employees, agents, and representatives, from any and all liability for injuries or damages arising from participation.
Facility Use Acknowledgment: I understand that Vista College Preparatory is providing facility space only and is not responsible for the supervision or operation of Canyon Academy FC programs.
Medical Authorization: I authorize Canyon Academy FC representatives to obtain emergency medical care for the participant if necessary.
Indemnification: I agree to indemnify, defend, and hold harmless Canyon Academy FC and Vista College Preparatory from any claims arising from participation.
Code of Conduct: Players must respect teammates, coaches, referees, staff, and club policies.
Media Release: Photos and video taken during camp activities may be used for club promotion.
